Act as integral IDT member coordinating social work services for frail seniors
Provide psychosocial assessments, care planning, counseling, and case management
Collaborate with family, participants, and care teams to support safe community living
Facilitate care transitions between settings and coordinate resources
Lead discharge planning when participants disenroll

🎯 Requirements

MSW required
One year of experience with frail or elderly population preferred
CPR and First Aid certification within 180 days preferred
Experience coordinating care conferences a plus
Strong ability to work within an IDT and with primary care teams
