Social Worker, Part-time

📋 Description

• Part-Time Social Worker to support elderly participants in PACE
• Coordinate with the IDT to ensure assessments and care plans are followed
• Deliver social work interventions, resources, and case management per care plan
• Support care transitions between settings and participate in care conferences
• Provide discharge planning when participants disenroll

🎯 Requirements

• Master’s degree in Social Work (MSW)
• Minimum 1 year experience with frail/elderly populations
• Experience coordinating care conferences
• Medical clearance for communicable diseases; up-to-date immunizations
• CA driver’s license, transportation, and insurance as required by law
• CPR/BLS certification or ability to obtain within 30 days
