Supportive Housing Case Manager

📋 Description

• Conduct assessments and interviews to determine eligibility.
• Develop and update action plans, case notes, and client files.
• Provide life skills development to improve independence and quality of life.
• Document care and follow support plans with strong reading and writing skills.
• Offer targeted interventions when clients risk eviction.
• Coordinate referrals to medical, mental health, and other services.

🎯 Requirements

• Bachelor's Degree in social services or related field and at least one year experience in housing
• High School Diploma or GED with two years of related experience
• Self-starter with good time management and proactive problem solving
• Flexible schedule, technology savvy (Microsoft Office, Zoom, PC, Android)
• Valid driver’s license, insurance, and vehicle access
• CPR/First Aid ability; ability to lift up to 50 lbs

🎁 Benefits

• Wage: $23.00/hr; full range $23.00 - $25.65 per hour
