Supportive Housing Case Manager

📋 Description

• Conduct assessments and interviews to determine eligibility.
• Develop and update action plans, case notes, and client files.
• Provide life skills development to improve independence and quality of life.
• Document care and collaborate with internal and external teams.
• Provide targeted interventions when clients are at risk of eviction.
• Connect clients to medical, mental health, and other services.

🎯 Requirements

• Bachelor's Degree in social services or related field and one year experience in housing
• Self-starter with good time management and proactive problem solving.
• Technology savvy with Microsoft Office, Zoom, PC and Android devices.
• Valid driver’s license, insurance, and access to a vehicle insured for you as a driver.
• Ability to perform CPR/First Aid; ability to lift up to 50 lbs.
• Must be at least 21; bilingual preferred.

• Wage: $23.00/hr; full range $23.00 - $25.65/hr with potential increases based on tenure and
