Collaborate with healthcare professionals to develop and monitor medication therapy plans
Monitor drug interactions, adverse effects, and therapeutic outcomes
Maintain accurate patient records and contribute to clinical protocols

🎯 Requirements

PharmD and NY Pharmacist license required
Strong clinical knowledge in TM program areas (diabetes and/or weight management)
Excellent communication and interpersonal skills
Ability to work as part of a healthcare team
Attention to detail and commitment to patient safety
Continuing education and professional development commitment
