Transition Coordinator

📋 Description

• Supervise students (group/individual) and monitor behavior per program expectations.
• Counsel students on personal, social, behavioral, or academic issues affecting progress.
• Lead IEP transition meetings; use assistive tech and person-centered tools.
• Navigate post-secondary goals versus family expectations; apply testing and job shadowing.
• Apply IDEA mandates; draft goals and pivot plans from assessment data.
• Track progress with digital tools, rubrics, and self-monitoring at work-based sites.

🎯 Requirements

• Bachelor’s degree strongly preferred.
• Background in education or social work preferred.
• Regulatory knowledge of IDEA transition mandates.
• Transition and coaching experience with students.
• Systemic mindset focused on real-world networks and resources.
• Behavioral and performance knowledge; strong written and verbal communication.
