Bridge school to adult life by fostering independence and community networks.
Lead IEP transition meetings using person-centered planning and assistive tech.
Manage transition assessments and IDEA compliance with progress tracking.
Develop work-based opportunities and maintain relationships with community sites and agencies.
Coach students on portals, accommodations, and professional relationships.

🎯 Requirements

Bachelor’s Degree strongly preferred.
Background in education or social work preferred.
Strong knowledge of IDEA transition mandates.
Transition & coaching experience in jobs and college.
Systemic mindset using community networks and transit.
Behavioral and performance knowledge (ASD, ADHD).
