Transition Coordinator

📋 Description

• Prepare students aged 14-22 for life after school; independence
• Bridge school to adult life; build community networks
• Support two Menta locations: Belleville and Millstadt IL
• Lead IEP transition meetings; use person-centered tools
• Develop work-based opportunities: internships and volunteer opportunities
• Cultivate connections with VR, Pre-ETS and related services

🎯 Requirements

• Bachelor’s degree strongly preferred
• Background in education or social work preferred
• Strong knowledge of IDEA transition mandates
• Transition and coaching experience in jobs and college
• Systems mindset; leverage community networks
• Behavioral and performance knowledge (e.g., ASD, ADHD)
