📋 Description

• Prepare students aged 14-22 for life after school.
• Bridge school-to-adult life via community networks.
• Support counseling, supervision, and behavior management.
• Lead IEP transition meetings with person-centered planning.
• Coordinate work-based learning, internships, and jobs.
• Collaborate with agencies (VR, Pre-ETS) and community partners.

🎯 Requirements

• Bachelor's degree strongly preferred.
• Background in education or social work preferred.
• Deep knowledge of IDEA transition mandates.
• Transition and coaching experience.
• Systems mindset with community networks.
• Knowledge of behavioral needs (e.g., ASD, ADHD).
