📋 Description

Start of Care/Oasis Admissions RN for Jacksonville, FL
Per visit basis service across Duval, Baker, Clay, Nassau Counties
Travel to client homes within designated territories
Follow designated care plans and administer medications
Conduct client assessments and document care interventions
Coordinate case management and document outcomes

🎯 Requirements

FL Registered Nurse license (current)
Minimum 1 year clinical (medical/surgical) exp.
Home health exp preferred; not required
Strong interpersonal skills and independent work style
Solid computer skills; EMR experience preferred
Ability to travel to cases as assigned
