Admissions Physical Therapist

📋 Description

• Complete OASIS Start of Care (SOC) admissions visits for home health patients throughout Burlington
• Perform comprehensive PT evaluations: mobility, strength, balance, gait, endurance, transfers
• Develop individualized treatment plans based on patient needs and physician orders.
• Collaborate with nursing and interdisciplinary team to coordinate care.
• Educate patients, families, and caregivers to promote independence and safety.
• Document clinical findings and services using EMR and tablet technology.

🎯 Requirements

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent experience as a Physical Therapist.
• Prior home health experience required.
• OASIS experience required.
• HCHB experience strongly preferred.
• Strong clinical, communication, organizational, and time-management skills.
