Admissions Registered Nurse

📋 Description

• Complete SOC and admission visits for home health patients
• Perform thorough patient assessments and OASIS documentation
• Develop individualized care plans with physicians and team
• Deliver skilled nursing care, meds, wound care, and education
• Monitor progress and communicate changes to care team
• Educate patients and caregivers for independence at home

🎯 Requirements

• Current New Jersey RN license in good standing
• Minimum one year nursing experience; home health preferred
• Experience with OASIS documentation and SOC visits
• Comfortable with EMR; HCHB experience is a plus
• Excellent assessment, communication, critical thinking, and organization
• Reliable transportation; willingness to travel throughout Ocean County
