Use head-to-toe assessment to identify changes in patient condition
Provide acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical)
Triage and manage urgent clinical situations in the home
Perform medication reconciliation, teaching, safety monitoring
Conduct comprehensive wound assessment, care, and documentation

🎯 Requirements

Current nursing license in good standing in the state of Pennsylvania
Graduation from an accredited and approved nursing program
At least one year of hands-on medical experience within the past two years with adult/geriatric
