📋 Description

• Identify, resolve, and prevent medication-related problems to ensure safe and effective

🎯 Requirements

• Doctor of Pharmacy (PharmD) and active Louisiana pharmacist required and Texas pharmacist license
• Minimum 3 years as a clinical pharmacist; PGY1/PGY2 residency required.
• Strong clinical judgment, communication, and process management.
• In-depth knowledge of pharmacology, FDA regulations, and chronic disease management.
• Proven ability to ensure legal compliance and deliver high-quality, detail-oriented care.
