📋 Description

• Provide assessment, planning, primary care, and continuous medical coverage.
• Collaborate with community physicians and IDT on discharge planning.
• Participate in IDT, communicate changes, coordinate 24/7 care.
• Maintain care of participants in nursing homes with regular visits.
• Rotate night and weekend on-call duties.

🎯 Requirements

• Residency in an ACGME-accredited program.
• Master’s or doctoral degree.
• Board certification in Internal Medicine or Family Practice.
• Geriatrics board certification preferred.
• Active California physician license without restrictions.
• Active CDS and DEA certifications.
