📋 Description

  • Serve as main contact for new participants and families during onboarding and journey.
  • Advocate for participants joining the PACE program; ensure seamless onboarding.
  • Facilitate care coordination from pre-enrollment to enrollment with IDT meetings.
  • Review participant tracking tools to identify immediate needs; coordinate with IDT.
  • Oversee monthly new enrollee orientation day, WelbeLink training, and center tour.
  • Conduct checkpoints at 30, 60, and 90 days post enrollment and ongoing follow-ups.
  • Partner with Behavioral Health and Social Work teams for high-risk participant check-ins.
  • Identify high-risk participants for disenrollment and address concerns.

🎯 Requirements

  • Bachelor’s degree in human services, communications, hospitality, or related field; experience may substitute.
  • 4+ years in customer service or customer-facing roles.
  • Highly motivated, self-directed; adapts quickly and makes decisions in emergencies.
  • Proficiency in data entry and data analysis.
  • Proficiency in Microsoft Office programs.
  • Ability to manage sensitive conversations with compassion and discretion.
