Behavioral Health Therapist

📋 Description

• Screen and assess patients for mental health conditions and substance use disorders, identify
• Conduct patient assessments and develop individualized treatment plans in collaboration with the
• Deliver brief, evidence-based behavioral health interventions, including behavioral activation
• Support psychotropic medication management prescribed by medical providers by monitoring adherence
• Provide health behavior interventions and address social determinants of health to improve daily
• Monitor patient symptoms and treatment response through in-person appointments, video

🎯 Requirements

• Master's degree or higher in Social Work, Psychology, Counseling, Rehabilitation, or another
• At least 3–5 years of experience in behavioral health clinical interventions, case management
• Experience supporting diverse patient populations, including older adults covered by Medicare and
• Active, independent clinical licensure in Oregon as one of the following: Licensed Clinical Social
• Eligibility to enroll in Oregon's Medicaid and Medicare programs; current enrollment is preferred.
