Bilingual Care Coordinator, LPN (Pacific Time)

📋 Description

• Monthly CCM check-ins with patients
• Identify and address clinical concerns
• Reinforce care plans and document accurately
• Escalate issues per triage pathways
• Support patients between visits via EHR/CCM tools
• Use SMS as first-line patient engagement channel

🎯 Requirements

• Active LPN or LVN license
• Bilingual Spanish/English
• 1-2 years in ambulatory care, population health, care coordination
• Strong clinical assessment, communication, and documentation
• Comfortable using two systems (EHR and CCM platform)
• Proficient with MS Office (Outlook & Teams)
