CA Licensed Bilingual Farsi/English Clinician - LCSW or LPCC - Behavioral Health

📋 Description

• Provides care to patients; tailor plans per Care Pod.
• Coordinates BH care with PCPs and other providers.
• Screens for BH and SUD using PHQ9, GAD7, CSSRS.
• Monitors symptoms and meds side effects; aims for improvement.
• Supports psychotropic med management by PCPs.
• Documents progress in Salesforce and EHR for sharing with providers.

🎯 Requirements

• Master's Degree in Social Work, Counseling, or related field.
• Licensed to practice in CA as LCSW, LPCC, or equivalent.
• Minimum 1-5 years experience as licensed professional.
• A secure, high-speed internet connection for video conferencing and cloud-based systems.
• Proficient in medical interpretation and documentation in English and Farsi.
