CA Licensed Bilingual Spanish/English Clinician - LCSW or LPCC - Behavioral Health

📋 Description

• Provides care via the Collaborative Care Model to pediatric and/or adult patients.
• Tailors treatment plans to individual patient needs.
• Coordinates behavioral health care with PCPs and other providers.
• Uses PHQ-9, GAD-7, CSSRS to screen for issues.
• Monitors symptoms and side effects to reduce symptoms.
• Documents progress in Salesforce and EHR for sharing with care team.

🎯 Requirements

• Master’s Degree in Social Work, Counseling, or related field.
• Licensed in CA as an independent clinician (LCSW/LPCC).
• Minimum 1-5 years experience as licensed professional.
• High-speed internet for video conferencing and cloud systems.
• Proficient in English and Spanish medical interpretation and documentation.
• Proficient with EHRs and new technology.
