CA Licensed Clinician - LCSW or LPCC - Behavioral Health

📋 Description

• Provides care to patients and customizes treatment plans.
• Coordinates behavioral health care with the patient’s PCP and other service providers.
• Screens for behavioral health and substance use with PHQ9, GAD7, CSSRS.
• Monitors symptoms and side effects to reduce symptoms.
• Supports psychotropic medication management by PCPs.
• Tracks follow-up and outcomes in a registry to re-engage patients.

🎯 Requirements

• Master’s Degree in Social Work, Counseling, or related field from an accredited program
• Licensed to practice in California as an independent clinician (LCSW, LPCC, or equivalent)
• Minimum 1-5 years experience as a licensed professional
• A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems
• Proficiency in English and Spanish medical interpretation and documentation
• Knowledge of/experience with behavioral health screening tools (PHQ9, GAD7, CSSRS)
