📋 Description

• Screen members for eligibility in the hypertension care program and support enrollment throughout
• Provide blood pressure monitors, assess members' measurement techniques, and reinforce accurate
• Conduct comprehensive hypertension assessments covering medical history, medications, adherence
• Deliver lifestyle and nutrition counseling focused on sodium and alcohol reduction, heart-healthy
• Identify barriers to healthy behaviors and coach members toward realistic, achievable, and

🎯 Requirements

• Experience working as a Medical Assistant or in a clinical or healthcare setting.
• At least 2 years of experience supporting patients through longitudinal chronic disease management
• Strong patient coaching and engagement skills, with the ability to build trust and communicate
• Experience assessing patient needs, identifying barriers to care, and supporting behavior change.
• Strong organizational skills and attention to detail for managing documentation, follow-ups, and
• Ability to collaborate effectively with multidisciplinary clinical and support teams.
