Care Coordinator

📋 Description

• Manage member caseloads in assigned geographic area.
• Coordinate face-to-face visits.
• Conduct annual assessments including PCA assessments for community members.
• Develop individualized care plans and goals with the care team.
• Connect members with community/state resources and services.
• Complete required documentation and support transitions of care.

🎯 Requirements

• Minnesota RN license or MN Licensed Social Worker
• 1+ year geriatric/case management experience
• Valid driver's license
• Ability to work independently
• Private home office space
• Strong verbal and interpersonal skills
