Care Coordinator

📋 Description

• Be the primary bridge and entry point for all members and providers working with the Care
• Field inbound phone calls from members and providers, using optimized IVR pathways to ensure
• Manage care coordination for members across various programs, including CCM, TOC, and unassigned
• Manage non-emergent medical transportation (NEMT) via UberHealth for members, strictly following
• Handle internal escalations to specialized departments such as Claims, Eligibility, and Care
• Lead special projects as subject matter experts.

🎯 Requirements

• 1+ years of experience with care coordination and navigation in a healthcare setting.
• Proficient computer literacy skills, with a strong aptitude for using EMRs (like Medecision) and
• Excellent interpersonal communication skills including exceptional listening skills.
