Care Coordinator, LSW (MSHO) – $2,500 Sign-On Bonus

📋 Description

• Care coordination for MSHO/MSC+ members in the community and assisted living.
• Work from home with 50-70% travel in the St. Paul, MN area.
• Coordinate face-to-face visits and annual assessments (PCA where needed).
• Develop individualized care plans and connect members with resources.
• Support transitions of care and collaborate to meet quality measures.
• Complete required documentation and maintain compliance.

🎯 Requirements

• Current MN Licensed Social Worker (new grads encouraged)
• Minnesota Registered Nurse license
• 1+ year geriatric/certified disabled case management experience
• Valid driver’s license
• Ability to work independently with a private home office
• Strong verbal and written communication skills
