📋 Description

• Coordinating face-to-face visits
• Managing the Elderly Waiver
• Conducting annual PCA assessments for community members
• Reviewing health needs, goals, and developing individualized care plans
• Connecting members with community/state resources and services
• Completing required documentation

🎯 Requirements

• Current MN Licensed Social Worker (new grads encouraged to apply)
• Minnesota Registered Nurse license
• 1+ year experience with geriatric or disabled population in case management
• Must have a valid driver's license
• Ability to work independently
• Excellent interpersonal and customer service skills
