Care Coordinator, LSW (MSHO)

📋 Description

• Manage member caseloads within assigned geographic area.
• Coordinate face-to-face visits and Elder Waiver participation.
• Conduct annual PCA assessments for community members.
• Review health needs, set goals, and develop care plans.
• Connect members with community and state resources; complete documentation.
• Support transitions of care with discharge and follow-up plans.

🎯 Requirements

• Current MN Licensed Social Worker (new grads encouraged to apply).
• OR Minnesota Registered Nurse license.
• One or more years of experience with geriatric or disabled populations in care coordination preferred.
• Must have a valid driver’s license.
