Care Coordinator, LSW (MSHO)

📋 Description

• Manage member caseloads within assigned geographic area.
• Coordinate face-to-face visits and Elder Waiver activities.
• Conduct annual assessments including PCA for community and assisted living.
• Develop individualized care plans and health goals.
• Connect members with community/state resources and complete documentation.
• Collaborate with care teams to meet quality measures and transitions of care.

🎯 Requirements

• Current MN Licensed Social Worker (new grads encouraged)
• Minnesota Registered Nurse license
• 1+ year experience with geriatric or certified disabled populations in case management/care coordination
• Valid driver's license
• Ability to work independently with a private home office
• Strong verbal communication and interpersonal skills
