Coordinating face-to-face visits
Managing the Elderly Waiver
Conducting annual assessments including PCA assessments for community members and living tools for Assisted Living
Reviewing health needs, identifying goals, and developing care plans

Completing required documentation

🎯 Requirements

Current MN Licensed Social Worker (new grads encouraged to apply)
OR Minnesota Registered Nurse license
One or more years of geriatric or disabled population case management experience
Must have a valid driver’s license
