Care Coordinator, LSW (SNBC)

📋 Description

• Manage member caseloads in assigned geographic area
• Coordinate face-to-face visits and assessments
• Develop individualized care plans and goals with members
• Connect members with community resources; complete required documentation
• Collaborate with medical care teams to meet quality measures and value-based goals
• Support transitions of care and follow-up planning

🎯 Requirements

• Current MN Licensed Social Worker (new grads encouraged)
• Minnesota Registered Nurse license
• 1+ year experience with geriatric or disabled populations in case management or care coordination
• Valid driver's license
• Ability to work independently
• Excellent verbal and written communication skills
