Care Coordinator Manager

📋 Description

• Lead, manage, and develop a team of Care Coordinators supporting patients from activation to
• Oversee patient support, engagement, and care coordination workflows
• Identify and manage escalations/step-up care with UHS facilities
• Monitor performance metrics, quality, and HEDIS-related measures
• Partner cross-functionally with Clinical, Quality, Product, and Network teams
• Develop standardized workflows and support product improvements

🎯 Requirements

• Active independent clinical licensure with 3-5 years clinical experience
• 3+ years in healthcare operations, care coordination, or related field
• Experience managing teams focused on patient outreach/engagement
• Strong understanding of patient care journeys and care transitions
• Experience improving operational workflows and using performance metrics
• Strong communication, organizational, and problem-solving skills
