Care Coordinator, RN (MSHO)

📋 Description

• Manage DHS and CMS duties for MSHO/MSC+ members in the community.
• Perform face-to-face visits and annual assessments with members.
• Develop individualized care plans and goals with the care team.
• Coordinate with community resources and services; document accordingly.
• Travel 50-70% locally; remote work 30-50%.
• Hours: 8am-5pm Mon-Thu, 8am-3pm Fri; no evenings or weekends.

🎯 Requirements

• Current Minnesota Registered Nurse license.
• MN Licensed Social Worker (new grads encouraged).
• 1+ year experience with geriatric or disabled population in case management.
• Valid driver's license.
• Ability to read/write/speak English.
• Excellent communication and customer service skills.
