Manage member caseloads within assigned geographic area
Coordinate face-to-face visits
Manage the Elderly Waiver
Conduct annual assessments, including PCA assessments for community members
Review health needs, identify goals, and develop care plans
Collaborate with medical care teams to meet quality measures and utilization goals

🎯 Requirements

Minnesota Registered Nurse license
OR Current MN Licensed Social Worker (new grads encouraged)
1+ year experience with geriatric/disabled populations in case management
Valid driver's license
Ability to work independently
Excellent interpersonal and customer service skills
