Care Coordinator, RN (MSHO)

📋 Description

• Manage DHS and CMS activities for MSHO/MSC+ members.
• Work from home with regular local travel.
• Travel ~50-70% in North Branch area; ~30-50% remote.
• Coordinate face-to-face visits and Elderly Waiver.
• Conduct annual PCA assessments for community and assisted living.
• Develop care plans and support transitions of care.

🎯 Requirements

• Minnesota RN license.
• MN Licensed Social Worker (new grads welcome).
• 1+ year geriatrics/certified disabled case management experience.
• Valid driver's license.
• Independent worker with private home office space.
• Excellent verbal, interpersonal and customer service skills; aligned with Bluestone.
