Conducting annual assessments including Personal Care Assistance (PCA) assessments and customized
Reviewing health needs, identifying goals, and developing individualized care plans
Connecting members with community and state resources
Completing required documentation

🎯 Requirements

Minnesota Registered Nurse license OR current MN Licensed Social Worker (LSW, LMSW, LISW, or LICSW)
One or more years of experience with geriatric or disabled population in case management/care
Valid driver's license
Ability to work independently with strong communication skills
