Care Management Pharmacist

📋 Description

• Collaborate with physicians and care managers to manage CKD meds.
• Coordinate with RNs, nurse care managers, health coaches, dietitians, and others.
• Provide patient education and optimize medication regimens.
• Support interdisciplinary pharmacy care programs across the care spectrum.

🎯 Requirements

• Doctorate or PharmD or bachelor’s degree in pharmacy.
• Unrestricted state license or within 60 days of hire.
• Strong pharmacotherapy knowledge focused on quality of life.
• Residency training in ambulatory care or related programs.
• Multistate licensure (TN, MN, CO).
• Experience in care management, value-based care, or population health.
