• 1:1 patient management in senior living communities.
• Primary liaison between patient, family, and PCP.
• Focus on closing care gaps and reducing readmissions.
• 90% field-based care across Minnetonka and SW Metro.

🎯 Requirements

• Bachelor's degree or higher; LPN/RN or Social Worker preferred.
• 3-5 years in value-based care, population health, or care coordination.
• Behavioral health experience preferred.
• Knowledge of Assisted Living and Memory Care communities.
• Independent work with strong time management and organization.
• Proficient with EHRs; strong written and verbal communication.
