Care Manager

📋 Description

• Field-based care manager providing 1:1 patient management in senior living communities.
• Primary liaison between patient, family, and PCP; ensures a unified care plan.
• Optimize outcomes, close gaps in care, reduce hospitalizations.
• Travel to Assisted Living/Memory Care/Group Homes across Minnesota, Wisconsin and Florida.
• Collaborate with MDs, NPs, and PAs; update care strategies as patient status evolves.
• Support transitions to/from inpatient care; prevent readmissions.

🎯 Requirements

• Bachelor's degree or higher preferred; LPN/RN/Social Worker preferred
• 3-5 years in value-based care, population health, case management, discharge planning
• Behavioral health experience preferred
• Knowledge of Assisted Living/Memory Care communities
• Strong time management and organizational skills
• Proficient with EHRs; strong written and verbal communication
