Care Manager

📋 Description

• Field-based care manager delivering 1:1 patient care in senior living communities.
• Primary link between patient, family, and PCP to execute unified care plan.
• Develop and manage individualized care plans; support advance care planning.
• Implement CoCM behavioral health interventions and collaborate with care team.
• Close care gaps, optimize transitions, and reduce hospital utilization.
• Full-time role with day shifts; 90% field-based across Southern Sarasota.

🎯 Requirements

• Bachelor’s degree or higher preferred; LPN/RN/Social Worker license preferred.
• 3-5 years in value-based care, population health, case management.
• Behavioral health experience preferred.
• Knowledge of Assisted Living and Memory Care communities.
• Strong time-management and organizational skills; able to work independently.
• Proficient with EHRs; strong computer skills (email, word processing, spreadsheets).
