Care Manager

📋 Description

• Schedule and perform quarterly supervisory visits for Home Care Compliance.
• Serve as field expert to support members and caregivers in care utilization.
• Perform home inspections to ensure safety and compliance.
• Monitor daily notes and document caregiver contacts in Salesforce.
• Maintain contact with caregiver per service plan and gather needs.
• Track activities in CRM and work within a fast-paced startup.

🎯 Requirements

• Bachelor's degree or equivalent work experience.
• 2+ years bedside CNA, QMAP, Direct Care Worker, Medical Assistant, etc.
• 1+ years in call center, telephonic outreach, customer service or similar.
• Experience with data input, case review, or Salesforce is a plus.
• Ability to exercise independent judgement and identify issues/trends.
• Strong communication, problem-solving, time management, and empathy.
