Care Manager for Bibb & Lamar, GA area with field-based responsibilities
Perform orientations, assess new patients, and train caregivers to ensure quality home care
Collaborate with care team, caregivers, and patients to support independence at home
Travel within the Georgia (Atlanta Metro) region to support clients

🎯 Requirements

2+ years of experience with frail or elderly disabled adults and caregivers
Previous field/home care, case management, or medical coordination experience preferred
Excellent communication, organization, and time management
Strong critical thinking, problem solving, and team collaboration
Hands-on experience in home care and ability to travel within GA
Proficiency in Microsoft Office; Salesforce experience a plus
