Care Manager (Colorado Springs, CO / Field-based)

📋 Description

• Field-based care manager role across the Denver, CO area with in-home patient care coordination.
• Schedule and perform quarterly supervisory visits for Home Care Compliance.
• Serve as field expert to support waiver members and caregivers in care planning and utilization of
• Conduct home inspections to ensure safety and compliance.
• Monitor Daily Notes with the care team and document caregiver interactions in Salesforce.
• Maintain caregiver contact and communicate health concerns to staff RN/Case Manager.

🎯 Requirements

• Bachelor’s degree or equivalent work experience.
• 2+ years of bedside care experience (CNA, QMAP, Direct Care Worker, Medical Assistant, etc.).
• 1+ years of call center, telephonic outreach, or customer service experience.
• Strong verbal and written communication; high empathy and ability to build rapport.
• Strong problem-solving, time management, and multitasking skills.
• Salesforce experience is a plus; familiarity with HIPAA guidelines.
