Care Manager (Columbus, IN / Field-based)

📋 Description

• Perform patient orientations and initial assessments
• Assess home care needs, environment, and safety
• Train caregivers to deliver high-quality home care
• Conduct supervisory visits and caregiver/patient check-ins
• Collaborate in interdisciplinary meetings and share case impressions
• Travel statewide within Indiana (mileage reimbursed)

🎯 Requirements

• 2+ years experience with frail or elderly adults and caregivers
• Field medical/case management experience preferred
• Data entry experience; Salesforce a plus
• Hands-on experience in home care preferred
• Knowledge of ADL/IADL assessments
• Valid driver’s license with insured vehicle for IN travel
