Care Manager (Dekalb & Rockdale, GA / Field-based)

📋 Description

• Perform orientations and assess new patients for our program.
• Train caregivers to ensure high-quality in-home care.
• Conduct home visits and contribute to ongoing patient care plans.
• Coordinate with caregivers, patients, and staff; travel within GA.
• Engage in interdisciplinary meetings and provide case updates as required.
• Wear multiple hats as operations develop and support the care practice.

🎯 Requirements

• 2+ years of experience with frail or elderly adults and caregivers
• Hands-on experience in home care, patient assessment, and caregiver training
• Excellent verbal and written communication; strong organizational and time management
• Proficient in Microsoft Word, Excel, Outlook; Salesforce experience a plus
• Valid driver’s license with insured vehicle for travel within GA
• Excellent teamwork and a team-player mentality
