Care Manager (Gwinnett & Lawrenceville, GA / Field-based)

📋 Description

• Perform orientations and assess new patients for our program
• Train caregivers to ensure high-quality home care
• Coordinate care with families and stakeholders
• Travel within the Gwinnett/Lawrenceville, GA area for field-based duties
• Support program goals and patient independence at home

🎯 Requirements

• 2+ years of experience with frail or elderly adults
• Hands-on in home care or case management
• Experience with Salesforce; strong data entry skills
• Excellent communication and organizational skills
• Proficient with Microsoft Office (Word, Excel, Outlook)
• Valid driver’s license; insured vehicle for travel within GA
