Care Manager II (Field-Based 4 x 10 schedule Monday through Friday)

📋 Description

• Conduct orientations and assess new patients for the program.
• Train caregivers to deliver high-quality in-home care.
• Schedule and conduct in-home visits; document health observations.
• Balance 32 hours of field visits with 8 hours of admin work (remote).
• Collaborate with care teams, patients, and caregivers; participate in interdisciplinary meetings.
• Travel within Missouri (Kansas City market) as required.

🎯 Requirements

• 2+ years experience with frail or elderly disabled adults and caregivers.
• Prior field experience in medical or case management environment preferred.
• Salesforce experience is a plus; experience with data input and case notes.
• Excellent verbal and written communication, organizational and time management skills.
• Proficient in Microsoft Word, Excel, Outlook.
• Valid driver's license and ability to travel within Missouri.
