📋 Description

• Conduct initial in-person clinical assessments and coordinate timely service activation, ensuring
• Identify potential changes to the individualized plans of care, making and documenting revisions as

🎯 Requirements

• Diploma from an accredited nursing program.
• Valid Licensed Practical Nurse license and in good standing by Missouri Board of Nursing
• Minimum of (1) year of experience with the care of the elderly, or individuals with disabilities or
• Valid driver’s license and car insurance required, with means of reliable transportation.
