Care Manager, RN

📋 Description

• Provide direct nursing care to participants in clinic, home, or center.
• Act as quarterback of the Interdisciplinary Team (IDT) for your panel.
• Conduct assessments and drive care plans across settings.
• Implement nursing interventions and med administration per plan.
• Participate in IDT meetings, sharing assessment insights.
• Deliver person-centered care; ensure safety and quality.

🎯 Requirements

• Unencumbered California RN license.
• Graduate of accredited nursing program; BSN preferred.
• 2-4 years RN experience with medically complex/older adults.
• Alzheimer's certification or case management certification preferred.
• Wound care, IV therapy, and medication management skills.
• Bilingual in Spanish preferred.
