Care Manager, RN (Moreland, Georgia/ Field-Based)

📋 Description

• Conduct initial and semi-annual in person visits, in-person RN supervisory visits and coordinate
• Complete self-scheduling for in person visits
• Complete a minimum of 16-20 in person visits weekly
• Complete in-person related visit documentation or care coordination the same business day
• Follow safety protocols including PPE use and HIPAA laws
• Drive independently to patient residences, up to 2 hours drive time

🎯 Requirements

• Diploma from an accredited nursing program; valid RN license in State of Georgia
• Minimum two years recent nursing experience with adults/geriatrics preferred
• Experience with Medicaid waiver programs, home and community-based services preferred
• Valid driver's license and car insurance
• Valid CPR/first aid certification
• Demonstrated commitment to high-level customer service
