Conduct initial in-person clinical assessments and coordinate timely service activation
Develop and manage individualized plans of care
Educate patients, caregivers, and internal teams
Coordinate patient care with physicians and community resources
Review caregiver documentation and respond to changes
Escalate clinical concerns and safety issues

🎯 Requirements

Diploma from an accredited nursing program
Valid and active RN license in Missouri
Valid driver's license and car insurance
Strong communication and problem-solving skills
Computer proficiency and documentation skills

🎁 Benefits

Base pay between $80,000 and $90,000 per year
